Cause and Manner Released in Death of Actress Hayden Panettiere

Actress Hayden Panettiere died from the toxic effects of fentanyl and several other drugs, according to new findings released by the Greenville County Coroner’s Office (GCCO), which has ruled her death an accident.

Panettiere, 36, was found unresponsive August 16, 2026, inside a residence on Easley Bridge Road in Greenville, where authorities said she had been temporarily staying.

The coroner’s office said the official cause of death was due to the toxic effects of fentanyl, 4-ANPP, alprazolam, methocarbamol and quetiapine. Her manner of death was ruled accidental.

The determination follows an autopsy conducted by a board-certified forensic pathologist. Investigators said the examination found no signs of trauma that would have contributed to Panettiere’s death.

The findings close the coroner’s investigation more than a month after Panettiere’s death – although that designation does not necessarily apply to any separate law enforcement investigations.

The emergency response

The investigation began on the afternoon of Sunday, August 16. According to the coroner’s office, a 911 call was received at approximately 1:51 p.m.EDT reporting a cardiac arrest at an apartment at Judson Mill Lofts on Easley Bridge Road.

Greenville police, firefighters and emergency medical personnel responded to the residence and found Panettiere in cardiac arrest. Paramedics began advanced cardiac life-support measures and attempted to resuscitate her.

Those efforts were unsuccessful.

Panettiere was pronounced dead at 2:32 p.m. EDT.

A Greenville Police Department (GPD) incident report (.pdf) said officers arrived as emergency crews were attempting lifesaving measures. The responding officer wrote that there did not appear to be an obvious physical reason for Panettiere to be unresponsive.

The GCCO responded to the scene and began what it described as a parallel investigation with the Greenville Police Department.

An autopsy was performed the following day.

At the time, coroner Mike Ellis said the examination revealed no injuries or trauma that contributed to Panettiere’s death. The cause and manner remained pending while investigators awaited additional studies, including toxicology testing.

As recently as last Monday (September 14, 2026) the coroner’s office stressed that no official cause of death had been released despite national media reports regarding preliminary toxicology information. Officials said at that time that the case remained pending until toxicology testing was complete.

This week’s findings represent the coroner’s official determination.

What the toxicology findings mean

The substances identified by the coroner include drugs from several different pharmaceutical categories.

Fentanyl is a powerful synthetic opioid used medically to treat severe pain. Illicitly manufactured fentanyl has also become widespread in the illegal drug supply. Opioids can suppress breathing, and an overdose can be fatal.

The toxicology findings also identified 4-ANPP, or 4-anilino-N-phenethylpiperidine. The substance is associated with fentanyl and is both an immediate chemical precursor used in fentanyl production and a recognized metabolite of fentanyl.

Alprazolam, commonly sold under the brand name Xanax, is a benzodiazepine prescribed for anxiety and panic disorders. Federal medical guidance warns that combining alprazolam with certain medications, including opioids, can increase the risk of severe sedation, breathing problems, coma and death.

Methocarbamol is a muscle relaxant that works by slowing activity in the nervous system and is commonly used to treat pain and discomfort associated with muscle injuries.

Quetiapine is an atypical antipsychotic medication used to treat conditions including schizophrenia and bipolar disorder and, in some circumstances, major depressive disorder.

The coroner’s finding attributes Panettiere’s death to the toxic effects of the substances collectively. The release does not specify the concentration of each substance, whether any particular drug alone would have been fatal or how each substance entered her system.

It also does not state that Panettiere intentionally ingested a lethal combination. The official designation of the manner of death as an accident distinguishes the death from suicide, homicide or natural causes.

Questions surrounded her death for weeks

Panettiere’s death drew national attention almost immediately, along with speculation about what happened inside the Greenville apartment before emergency crews were called.

A redacted GPD report identified Panettiere’s on-and-off boyfriend, Brian Hickerson, as the complainant. Hickerson and his brother, Zach Hickerson, were present as emergency crews attempted to resuscitate her.

Some national outlets subsequently reported purported details about Panettiere’s toxicology results before the coroner had completed the investigation. Her representative cautioned that those reports were unverified, while the coroner’s office maintained that no official cause or manner of death had yet been determined.

The newly released findings provide the first official answer: Panettiere died from the toxic effects of multiple substances, and her death was accidental.
